What Is All-on-4?

All-on-4 is a full arch implant technique developed by Nobel Biocare. Four titanium implants are placed in the jaw - two at the front in an upright position, and two at the back angled at 45 degrees. A fixed bridge carrying 10 to 14 prosthetic teeth is attached to these four implants.

The angled placement of the rear implants is the key innovation. It allows the implants to engage more bone at the back of the jaw, where bone loss is typically most significant, and in most cases eliminates the need for bone grafting before placement.

On the day of surgery, any remaining teeth are extracted and a temporary fixed bridge is typically attached to the implants. You leave the practice with teeth - not a gap, not a denture. The temporary bridge stays in place for 3 to 6 months while the implants heal and fuse with the bone. Once healing is confirmed, the final permanent bridge is fitted.

The single biggest difference beyond appearance and comfort is bone preservation. When teeth are missing, the jawbone that once supported them gradually resorbs (shrinks). This is why long-term denture wearers develop the characteristic sunken facial appearance over decades. Implants replace the tooth root and stimulate the bone, maintaining its volume and preserving facial structure.

All-on-4 vs Individual Implants

All-on-4 is not always the right solution. For patients missing several but not all teeth, individual implants for each missing tooth may be more appropriate. All-on-4 is most suitable when:

  • All teeth in an arch are missing or need to be extracted
  • Bone volume is limited (particularly at the rear of the jaw)
  • A faster or more cost-effective full-arch solution is the goal

Individual implants per tooth produce the most tooth-like result but require more implants and therefore greater bone volume and greater overall cost for a full arch.

The All-on-4 Process

Consultation and Planning

A 3D CBCT (cone beam CT) scan of the jaw is essential for planning All-on-4. It shows exact bone volume, density, and the location of nerves and sinuses - all critical for precise implant placement. Treatment planning software allows your dentist to plan the exact implant positions digitally before surgery.

Surgery Day

Under local anaesthesia (with sedation available for anxious patients), remaining teeth are extracted and the four implants are placed. A temporary fixed acrylic bridge is then attached. You go home with functional teeth.

Healing Phase (3 to 6 months)

The implants undergo osseointegration - fusing with the surrounding bone. You are on a soft food diet during this period. Regular check-ups monitor healing progress.

Final Bridge Placement

Once healing is confirmed, impressions (or a digital scan) are taken for your permanent bridge. Materials options include:

  • Acrylic on titanium frame: Lower cost, lighter weight, easier to repair
  • Zirconia: Stronger, more stain-resistant, most natural-looking, longer-lasting

The final bridge is attached to the implants. You walk out with your permanent smile.

All-on-4 requires diligent ongoing care:

  • Daily brushing with a soft brush along the gum line
  • Use of a water flosser or interdental brushes to clean under the bridge
  • Regular professional cleaning (twice yearly minimum) - specialist tools are needed to clean implant-supported bridges
  • Annual check-ups to assess implant stability and bone levels

With proper care, the implants are designed to last a lifetime. The bridge on top may need replacement after 15 to 20+ years depending on the material.
